Protective effect of proton pump inhibitor against gastrointestinal bleeding in patients receiving oral anticoagulants: a systematic review and meta-analysis

چکیده
Abstract Background The concurrent use of proton pump inhibitor (PPI) in oral anticoagulant (OAC) treated patients may be associated with a lower risk gastrointestinal bleeding (GIB), but evidence is still conflicting according to individual OACs. Purpose We conducted meta-analysis estimate the GIB OAC and PPI co-therapy. Methods A systematic search PubMed, EMBASE, Cochrane was performed for studies reporting Primary outcomes were total major events. calculated pooled estimates by random-effect reported as odds ratios (OR) 95% CI. Stratified analyses origin GIB, ethnic groups, OACs, presence underlying factors performed. Results 10 (1 randomized controlled study 9 observational studies) 1,970,931 who received included. co-therapy GIB; OR (95% CI) 0.67 (0.62–0.74) 0.68 (0.63–0.75) respectively. Among only upper (OR 0.67, CI 0.64–0.70). No difference primary observed between Asians non-Asians (p-for-difference, GIB=0.695, GIB=0.748, respectively) among OACs except edoxaban. protective effect on more significant high-risk patients, defined those medication antiplatelets or non-steroidal anti-inflammatory drugs 0.62, 0.52–0.73) high such previous history, HAS-BLED score ≥3, diseases. 0.65, 0.61–0.70). Conclusion In receive OAC, irrespective group type can considered particularly concomitant NSAID antiplatelet factors. Funding Acknowledgement Type funding sources: None.

New oral anticoagulants increase risk for gastrointestinal bleeding: a systematic review and meta-analysis.
BACKGROUND & AIMS A new generation of oral anticoagulants (nOAC), which includes thrombin and factor Xa inhibitors, has been shown to be effective, but little is known about whether these drugs increase patients' risk for gastrointestinal bleeding (GIB). Patients who require OAC therapy frequently have significant comorbidities and may also take aspirin and/or thienopyridines. We performed a sy...

OBJECTIVES To review randomised controlled trials of treatment with a proton pump inhibitor in patients with ulcer bleeding and determine the impact on mortality, rebleeding, and surgical intervention. DESIGN Systematic review and meta-analysis. DATA SOURCES Cochrane Collaboration's trials register, Medline, and Embase, handsearched abstracts, and pharmaceutical companies. REVIEW METHODS ...
متن کاملThe impact of bleeding complications in patients receiving target-specific oral anticoagulants: a systematic review and meta-analysis.
Vitamin K antagonists (VKAs) have been the standard of care for treatment of thromboembolic diseases. Target-specific oral anticoagulants (TSOACs) have been developed and found to be at least noninferior to VKAs with regard to efficacy, but the risk of bleeding with TSOACs remains controversial. We performed a systematic review and meta-analysis of phase-3 randomized controlled trials (RCTs) to...
متن کاملPerioperative management of patients receiving oral anticoagulants: a systematic review.
BACKGROUND The safety and efficacy of various management strategies for patients receiving oral anticoagulants (OACs) who need to undergo surgery or invasive procedures are unknown. METHODS We performed a systematic review and synthesis of the English-language literature examining the perioperative management and outcomes of patients receiving long-term OAC therapy. RESULTS Thirty-one repor...
متن کاملProton pump inhibitor alone vs proton pump inhibitor plus mucosal protective agents for endoscopic submucosal dissection-induced ulcer: a systematic review and meta-analysis
Mucosal protective agents may improve healing of patients with endoscopic submucosal dissection-induced ulcers. The present study systematically evaluated published clinical trials to determine whether combined therapeutic use of mucosal protective agents and proton pump inhibitors can improve the outcome of patients with endoscopic submucosal dissection-induced ulcers compared to treatment wit...
